Title: Do I need to fill out a form as I will be attending under 18?

I'm 17 and I don't know if I need to fill out anything to hand in.

Charlie, sorry to say this but you're not allowed to attend, you have to be over 18. This, unfortunately got overlooked due to a mistake on my part.

Please see your google checkout invoice, which has been refunded.
